Lithium orotate is a compound made of two parts: lithium and orotic acid. Only about 3.8-4% of the total weight is elemental lithium. The rest is the carrier molecule. That means a "5 mg lithium orotate" capsule delivers about 0.2 mg of actual lithium.
The problem is that supplement labels are inconsistent about what they're measuring.

Other brands list elemental lithium: "Lithium (as lithium orotate) 5 mg" = 5 mg elemental lithium, from roughly 120 mg of the compound
These are not the same dose. The second product contains approximately 25 times more elemental lithium than the first, even though both say "5 mg" on the front of the bottle.
Before comparing products or choosing a dose, check whether the label is reporting the compound or the element. Look at the Supplement Facts panel, not just the front label. If it says "Lithium (as lithium orotate)," the number refers to elemental lithium. If it says "Lithium orotate," the number refers to the full compound.

Can You Take Too Much Lithium Orotate?
In theory, yes. Lithium is dose-dependent: the higher the dose, the greater the risk of side effects. At prescription doses (600-1,800 mg of lithium carbonate per day), side effects including nausea, tremor, and increased thirst are well documented, and regular blood monitoring is required.
At supplemental doses, these effects have not been reported in published literature. But "supplemental dose" covers a wide range. Someone taking 20 mg of elemental lithium per day from a high-dose supplement is taking 100 times what Mentaid provides, and while that's still far below prescription levels, it's meaningfully different from a conservative trace-level dose.
